Perpignan captain Freshwater has been cited for allegedly "striking with the arm of London Irish flanker Kieran Roche".
Roche was let with a suspect broken cheekbone and was unable to see out of his left eye following an incident in the match at Stade Aime Giral on Saturday.
Having reviewed the match, citing commissioner John West has referred the incident to disciplinary chiefs - with Freshwater facing charges of striking or punching and acts contrary to good sportsmanship.
