Graeme Souness, Jamie Redknapp and Thierry Henry are on hand on Monday for a live look back at the Premier League season.

Monday’s Live Ford Football Special will take place inside Wembley Stadium after the Championship play-off final between Middlesbrough and Norwich, which is live on Sky Sports 1 from 2pm.

After finding out who will be the 20th team in the Premier League next season, the crew will look back on this year’s campaign, which has seen plenty of twists and turns.

Jose Mourinho guided Chelsea to their first Premier League title in five years as they secured top spot with three games to spare, leaving former champions Manchester City in their wake.

The relegation battle has proved to be as dramatic as ever, with Hull and Newcastle fighting to avoid joining Burnley and QPR in the Championship next season.
