The Gouden Gids Divisie leaders want the 22-year-old who has struggled to break into the Frisians' first-team.
Den Bosch want the attacker on loan until the end of the season and Den Bosch manager Fred van der Hoorn conceded talks had been held with the player's agent.
"The ball is with Kolk right now, his agent and Heerenveen," said van der Hoorn.
"We know the possibilities and we also know the limitations of our club.
"We will take big steps to make the transfer complete.
"It is a beautiful opportunity and I read in the paper the boy wants to come to us."
