Despite the fact the competition is entering its final stages the reigning Aviva Premiership champions have still left out a number of first-choice players.
Wing Tom Williams and scrum-half Karl Dickson are the only backs to retain their positions after the 27-16 loss to Exeter in the league last time out.
The back row of Luke Wallace, Maurie Fa'asavalu and Tom Guest remains unchanged and Mark Lambert stays at prop, but elsewhere it is all change.
Charlie Walker is named among the replacements and will make his first senior appearance of the season if he comes on.
Bath have opted to field their big guns, making just three changes - all in the front row - to the side which defeated Gloucester in the Aviva Premiership last weekend.
Props Nathan Catt and Anthony Perenise coming in for Paul James and David Wilson, who are both on international duty, and Ross Batty replaces the injured Rob Webber at hooker.
Club captain Stuart Hooper returns from injury to take a place among the replacements, while Argentinian international hooker Eusebio Guinazu could make his first appearance for Bath if he comes on.
