Do you still play 11-a-side football - or have you, like many, given it the boot? Either way, we want to hear from you.

We want your views on the state of grassroots football in the UK and Ireland.

The Premier League is as exciting as ever and there's a World Cup on the horizon, so why have participation levels dropped?

The latest FA figure's show more than 2,000 teams have folded since 2011 - that's more than 22,000 players.

We'd like you to take part in our grassroots football survey - so we can get to the root of the problem.
