Moore had a bogey-free 65, while de Jonge birdied four of the last five for a 66 to join them on 19-under.
American duo Jimmy Walker and Tim Herron are five strokes off the pace as the final trio surged away from the chasing pack.
Blixt had begun the back nine with a bogey, but the Swede's birdie at the par four 12th started the red-hot run, culminating in a 28-foot birdie putt at the 18th.
Zimbabwe's De Jonge, like Blixt seeking his first US PGA title, produced a similar streak until he narrowly missed a 12-foot birdie effort at 18.
Moore moved alongside with birdies at 15 and 16, but fellow American John Daly had a day to forget as he carded a 15-over 86, featuring a quadruple bogey, to slip from an overnight sixth to last.
"I was just hoping to stop the bleeding," Daly said. "I can't remember when I had such a stretch of bad holes."
