Ajax secured their first Eredivisie title since 2004 after beating FC Twente 3-1 in a winner-takes-all clash in Holland on Sunday.
FC Twente needed just a draw against Ajax but the visitors at the Amsterdam ArenA were undone when Siem de Jong broke the deadlock on 23 minutes.
A Denny Landzaat own goal doubled Ajax's advantage before Theo Janssen pulled a goal back two minutes into the second half.
De Jong, though, struck once more to cap a memorable day for manager Frank De Boer and his team.
De Graafschap finished the season in 14th, avoiding the relegation play-offs, after recording a 1-0 triumph over VVV-Venlo thanks to Rydell Poepon's sixth-minute goal.
PSV Eindhoven ended in third, two points behind the runners-up, after they played out a goalless draw versus Groningen.

A hat-trick from Dries Mertens ensured FC Utrecht capped off their campaign in style as the club thumped high-flying AZ Alkmaar 5-1.
Willie Overtoom helped himself to a brace while Samuel Armenteros also struck as Heracles Almelo slayed Ado Den Haag 3-0.
Excelsior will play in this season's relegation play-offs despite a 4-1 triumph at fellow strugglers Vitesse.
Guyon Fernandez, Tim Vincken and a brace from Geert Arend Roorda sealed the success while Marti Riverola bagged a consolation for Vitesse.
Feyenoord came from a goal down against NEC to salvage a point as Luc Castaignos cancelled out Lasse Schne's opener.
Elsewhere, Roda JC and Heerenveen played out a goalless stalemate while Julian Jenner came off the bench to score an 88th minute winner for NAC Breda at Willem II.
