The Italian side managed to overcome last year's Champions League winners thanks to a single goal from Ukrainian superstar Andrei Shevchenko at Guiseppe Meazza.
Portuguese playmaker Manuel Rui Costa played a superb ball through for Shevchenko five minutes before half time and he fired past Iker Casillas in Real's net from the edge of the box.
Madrid were unable to get back into the game and it was the Italian's that take the advantage in the battle between the group's favourite's.
The result was just the latest blow for the struggling Spanish side and Vicente Del Bosque will be hoping that he can turn things around for the next game in Group C.
Milan will now be early favourites to top the group and an early victory over Real will be a welcome boost to their hopes of lifting the trophy.
In the other group game, the Russian's took the lead at The Lokomotiv Stadium but Matthias Sammer will be delighted to get an early three points on the board after coming from behind to win 2-1.
The hosts took the lead just after the half hour mark when Ruslan Pimenov held the ball up seupebly before laying it to Sergei Ignashevitch to fire past Jens Lehman in Dortmund's net.
Dortmund went in search of an instant reply and got one just two minutes later when Thomas Rosicky crossed for Torsten Fings to head in to level the scores at 1-1.
Dede then crossed two minutes before the interval and the ball found it's way through to Jan Koller to pounce from close range.
The German side then held on to claim a well-earned early three points in a tough looking Group C containing Real Madrid and Milan.
