Aaron Downes (23) gave The Spireites the lead before Leon Constantine (44) levelled for the home side.
George Pilkington (77) gave Vale the lead before Caleb Folan (85) looked to have earned a point for the visitors.
However, Constantine proved Vale's hero by striking the winner two minutes later.
Nottingham Forest kept pace after coming from a goal down to defeat ten-man Brighton & Hove Albion 2-1 at The City Ground.
Jake Robinson (39) handed The Seagulls the lead, but a second-half brace from Grant Holt (pictured) - the first a penalty - gave Forest the win, as Brighton's Adam El-Abd was sent off for a second booking in the dying stages.
Bradford City are third after a convincing 3-0 victory at winless Crewe Alexandra.
Strikes from Eddie Johnson (44, 57) and Marc Bridge-Wilkinson (52) gave The Bantams a good away win in Cheshire.

Jo-Osei Kuffour (70) gave The Bees the lead before Danny Schofield (72) levelled for the visitors.
